KPMG, one of the Big Four accounting/management firms, issued a hallucination-filled report on AI

The report was issued in October, and it was discovered only recently that it "made numerour false claims" about current AI use by Swiss bank UBS, Britain's National Health Service, and public transit groups in Switzerland and London. The report has been pulled from KPMG websites.

KPMG is investigating to try to find out how this happened, but the report has already been distributed in several different countries and referenced by other industry publications.
