Or some enterprising soul could equip ships with all the needed supplies and equipment, park them in international waters, and vaccines become a boat ride away. A doctor in Europe did this for abortion services when it was illegal. The organization Mercy Ships provides medical care, including surgeries, for poor people in various ports in Africa.
